Where to Specify AuthID?

OK, this is probably an obvious question, but I hunted the tutorials, forums, and stared at the screen for half hour... to no avail

I am trying to register with a provider that allows DID. On my Sipura ATA I would enter UserID, AuthID, and password (AuthID is not the same as UserID). In Voxalot Voce Service Provider screen I have Username and "From User". Do they correspond to UserID and AuthID? Am I missing something obvious?

Username = AuthID
From User = UserID
