Many thanks to Martin for spending a great deal of time testing and interacting with me until we fixed the problem with incoming calls. We ended up disabling port forwards and enabling STUN. We tried audio optimisation as well but that resulted in one way audio with Pennytel so we had to revert to Voxalot handling the symmetric NAT.
I think the problem was that, at some stage, I had enabled STUN because I had read that this was a good thing. However, I had left port forwarding in place and this was interfering with the STUN settings.
