Provider's been removed, but...

I had one of Betamax clones as a Voice Service Provider for outbound calling. A couple of weeks ago I removed it from the Providers list. I also removed all Smart Dialing plans related to this provider.
But I've just found out that my calls are routed through this "ghost" provider following nonexistent dialing rules.
Is there a way to expunge those settings?

the us server must have a probelm. try au/eu/voxalot.com.
