As for providers, if there is at least one rule for each provider you are fine, no need to replicate any other rules

Arrange your priorities like this (just change the priority numbers):

1. Rules based on CID
2. Rules based on Provider (one per provider)
3. An ANY rule for all other calls (which will end up covering mostly calls directly to VoXalot, unless a CID rule applies)

As for VirtualTollFree, there is a crude way to route these calls, see the incoming CID to your end for when someone uses Virtual Toll Free, is your own VoXalot number, so setting a CID rule for _123456 (where 123456 is your VoXalot number will take care of all Virtual Toll Free Calls).
