Default Configuring Provider Settings

I need some help. When I log in and attempt to add my voice service provider there is no field for "Password" (or Confirm Password) in the Voice Service Provider form. What am I missing?

That's weird. When you add a provider, there should be two fields called Password and Confirm Password (see screenshot 1).

When you update a provider, these two fields are in a separate section (see screenshot 2). Click on the Change Provider Password button and the new section will appear.
