Priority was the issue!
I thought I configured it right, but here is my dial plan for a test I made for "mike":
201 _9675 001202XXXXXXX Voipcheap Yes
I also tried to change the number to 6 digits '_967522', thinking that a 4 digit number would cause problems.
In one test I even added a dot '_9675.' to see if it would behave differently.
I was getting a 403 Forbidden error.
SOLUTION: I moved the standard '_XXX.' pattern after this custom dial plan (lower priority), now it works!
