I have MNF and PT as options. Mostly everything goes to MNF and Mobile calls to PT.

I have:
Pattern: _04XXXXXXXX
Replacement: ${EXTEN}
Provider: PT
Active: Yes
ENUM Lookup: 614${EXTEN:1}
